This document is an integral part of the Public Offer (hereinafter — the Agreement). Terms are used with the meaning defined in the Agreement. Completing this questionnaire is a free, mandatory condition for activating the Partner’s access to paid domain name operations and to the interaction interface (API/EPP). Access is activated only after the questionnaire has been reviewed and confirmed (approved) by the Registrar; the Registrar has the right to refuse confirmation or to request additional information. The questionnaire is completed by any Partner regardless of its organisational and legal form (legal entity, individual entrepreneur or natural person). The Partner bears full responsibility for the accuracy and currency of the information provided; the provision of inaccurate information is grounds for refusal of confirmation, suspension or termination of the provision of Services. If the Partner’s activity requires registration as a business entity under the law, ensuring such status and paying taxes is the exclusive responsibility of the Partner.

By completing and submitting this questionnaire, the Partner confirms the accuracy of all the information provided and its status as a business entity, and also grants consent to the processing of the said data by the Registrar for the purpose of performing the Agreement. The questionnaire is submitted through the personal account, by e-mail to the Registrar’s address for official notifications, or through the electronic document flow service (Vchasno/Paperless) using a QES.
